Application for Enrollment

I hereby request space for my child _____________________ aged ______ for the program specified below. I enclose a $100.00 non refundable registration and processing fee. Upon acceptance of my child to the school, I will pay one month non-refundable fee as deposit. Tuition deposit will be applied towards the last payment with thirty days advance written notice as long as the child is attending school at the time the notice is given. If thirty days advance written notice is not given, the deposit will not be applied towards the last tuition payment.

FAMILY DISCOUNT

A 10% discount will be applied to a sibling's full day toddler/preschool tuition.

TERMS OF PAYMENT

All payments are due on the first of each month.
$300 materials fee for Kindergartners/Elementary children is due upon acceptance. Material fee is non-fundable and not prorated. Books will be issued only after the child graduates from kindergarten or Elementary program.

ABSENCE

No credit or refund is given for illness or trips out of town. The school's expenses continue, regardless of attendance.

LATE FEES & SERVICE CHARGES

Tuition is due on the first of the month and is late on the fifth. A late fee of $25 is added to the balance after the fifth of the month. An additional 1.5% service charge is charged every ten days thereafter. Students with unpaid tuition are subject to suspension unless other arrangements are made prior to that time. A $30.00 fee is charged on all checks returned by the bank.

Changes in a child's program during the school year will not be accepted.

WITHDRAWAL REQUIRES ADVANCE NOTICE

If a child is withdrawn during the School Year, his enrollment continues for thirty calendar days after WRITTEN notice is received at the office. Full payment for the last month is due with the written notice. 

Non-refundable deposit will not be applied towards the last month's payment for notice received after April 1, 2008 for the school year Sept. 2007 - June 2008.

Centerville Montessori reserves the right to terminate the enrollment of any child who, in its judgment, is not benefiting from the school. If enrollment is terminated by the school, tuition is prorated through the last day of attendance.

The state licensing agency has the right to inspect and review any child's file. When necessary the agency may interview a child or parent with or without a representative from the School.

I give release for the school to photograph or video my child at school for the purpose of school display, brochures, or seminars conducted in order to promote Montessori education.

I have read and accept the terms concerning holidays, payments, service charges, deposits, withdrawal and termination.
